SERVICES PROVIDED

Youth:

  • Initial and Ongoing Assessment of the youth’s emotional, educational, behavioral, nutritional, and medical needs (independent living assessment if age appropriate)
  • Comprehensive Social History
  • Comprehensive Treatment and Service Plan, development and implementation, including treatment goal development
  • Face-to-Face Assessment (frequency and intensity determined by level of care) in order to monitor progress towards treatment goals, provide counseling, and education if needed
  • Contact with collateral resources in order to provide “Continuity of Care” (frequency and intensity determined by level of care)
  • Provide Monthly reports, Quarterly reports, and Quarterly CTSP updates to referral source
  • Attend and report progress, goals, treatment services to FAPT/CAT, court reviews
  • Coordinate and lead quarterly Treatment Team meetings
  • Monthly Educational Peer Group counseling
  • Assist foster parent/youth in transportation if necessary
  • Respite Care (determined by level of care)
  • 24 hour on call crisis management/support services

Biological Families:

  • Consultation
  • Training (either individual, or through pre-service and ongoing if appropriate)
  • Comprehensive Family/Parenting Assessment, utilizing, but not limited to the following:  Ecomap, Family Life Cycle, Structured Family Assessment (assess relationships, issues of power/control, roles, and boundaries), Genogram, Descriptors, Bio/Psycho/Social Assessment, Strengths and Needs Assessment, Family System Analysis, and Recommendations.  Note:  this assessment will be completed if deemed appropriate by referral source and/or youth’s permanency goal
  • Involvement in Treatment planning for youth, and updated of progress when appropriate.  (Frequency/intensity determined by Family/Parenting Assessment and youth’s level of care)
  • Coordinate/Supervise visitation between youth and biological family (as determined by DHS or court)

Foster Families:

  • Foster/Adoptive parent Pre-Service and Continuing Training
  • Supervision, Education, Support, and Guidance in implementation of CTSP
  • 24 hour on call crisis management/support services
  • Individual/Group Counseling
  • Specialized Training (if needed) including utilizing outside and community resources
  • Pre-Placement counseling/consultation (reviewing documentation with staff to determine if a match and placement would be successful)
  • Short Term Foster Care (determined by youth’s level of care)
